To request a copy of a motor vehicle crash report, please complete the request form below, or call our administrative line at (608)267-1110 during business hours. You will need:

  1. Driver’s or involved party’s last name

  2. Date of incident

  3. Report number

Please allow 7-10 days after the date of the incident for crash reports to be ready for release.



Crash Reporting in Shorewood Hills

On 3/19/24 the Shorewood Hills Board of Trustees directed the SHPD to post all DT4000 crash reports on the SHPD website. Crash reports can provide valuable information for those working on traffic safety initiatives and are intended to provide insight into where, how, and why crashes occur in our community. All personally identifiable information of those involved, witnesses, and property owners will be redacted. Crash reports that pertain to open cases will not be posted. Cases may be in “open” status due to a variety of circumstances, such as awaiting blood alcohol/drug test reporting (currently up to a year turnaround), awaiting charging decisions from the District Attorney’s Office, or awaiting reconstruction or other reports from outside agencies. Only crash reports for incidents that occur within the Village of Shorewood Hills will be included in these postings.


Community Maps - WI Crash Data

Community Maps provides crash data from Wisconsin's law enforcement agencies and county Traffic Safety Commissions in a searchable statewide map of all police reported motor vehicle crashes from 2010 to the current year. Crashes are updated on a nightly basis using geo-coded locations from the Wisconsin Department of Transportation DT4000 police crash report. Community Maps is hosted at the University of Wisconsin-Madison by the Wisconsin Traffic Operations and Safety (TOPS) Laboratory in collaboration with the Wisconsin Department of Transportation (WisDOT) Bureau of Transportation Safety (BOTS).
